I hereby agree to understand the following contents and to receive appointments, treatments, and medication for stem cell therapy at Kinshukai Medical Corporation Infusion Clinic.

About Stem Cell Therapy

• ・This is a self-pay medical service, and the Japanese medical insurance system does not apply. The treatment cost will be fully self-funded.
• ・You will only receive treatment if you have understood the explanation of the treatment details and risks, and have agreed to it by your own will. (A separate consent form is required.)
• ・The consent form will be provided on the day of treatment or in advance, and you will be asked to sign it after a thorough explanation from a doctor.
• ・Even after you have signed the consent form, treatment may not be possible based on the doctor's judgment.

Patients who cannot receive treatment

1. ① Patients with hypersensitivity to anesthetics used during tissue collection or substances used in the manufacturing process.
2. ② Patients who have had an allergic reaction to penicillin, streptomycin, or amphotericin B used during cell culture.
3. ③ Patients with a positive result from a pathogenic microorganism test (HIV, syphilis).
4. ④ Minors.
5. ⑤ Pregnant or breastfeeding women.
6. ⑥ Cases of severe trauma where healing is not expected.
7. ⑦ Patients whom the doctor determines have a poor understanding of the treatment.
8. ⑧ Severely obese individuals with a BMI of 40 or higher.
9. ⑨ Patients with abnormal PT APTT results in pre-operative tests.
10. ⑩ Patients for whom it is difficult to obtain consent.
11. ⑪ Patients who have been given a life-expectancy prognosis.
12. ⑫ Individuals unable to walk independently (in some cases, treatment may be possible based on the doctor's judgment; please inquire via the contact form for details).

About the Agent Company

• ・Communication with patients is entrusted to an agent company that our clinic is affiliated with (EVRON Co., Ltd.).
• ・Inquiries made through our website will be shared with the agent company, who will then contact the patient.

About Appointments

• ・Appointments are made through the agent company.
• ・We may not be able to accommodate your desired appointment date.
• ・Any changes to the appointment date and time after confirmation will be treated as a cancellation.
• ・An appointment does not guarantee treatment. A doctor may decide that treatment is not possible.

About Payment

• ・Treatment fees are paid through the agent company.

Handling of Personal Information

• ・Personal information acquired for treatment purposes will be properly managed and protected in accordance with our clinic's regulations on handling personal information.
• ・In the following cases, we may inform an external third party of the patient's personal information.
• ・When information sharing with external parties is necessary to provide appropriate medical services.
• ・When information sharing is necessary for commissioned specimen testing or other business outsourcing.
• ・Appointment and payment procedures are handled through the agent company. The patient's personal information (necessary items such as name, date of birth, contact information, passport number, and medical information) will be provided to and shared with the said agent company for the purpose of smooth procedures and the provision of treatment services.

I agree to the cancellation fees.

• ・Cancellation fees will be charged in the following cases. Details on the specific fees and prices will be provided by the agent company at the time of your inquiry.
  1. ① Cancellation or rescheduling within one month of the fat collection date.
  2. ② Cancellation on the fat collection date.
  3. ③ Cancellation from the day after the fat collection date until the day before the stem cell administration date.
  4. ④ Cancellation on the stem cell administration date.
